Lake Wales 16 Year Old Killed In Crash

Lake Wales, Florida – The PCSO Traffic Unit is investigating a traffic crash that resulted in one fatality this morning in Alturas. Preliminary information so far is as follows:

Around 6:40 a.m. this morning (Thursday, December 29, 2016), a 1999 silver Chevrolet Silverado pickup truck was on Snell Road, crossing over Alturas Babson Park Cutoff (ABC) Road when it entered into the path of a 2011 white Chevrolet 3500 flatbed truck that was traveling westbound on ABC Cutoff Road. Both vehicles then spun together and struck a tree before coming to a final rest.

The driver of the Chevy Silverado pickup truck, and its sole occupant, 16 yr old Bailey (male) Tilley of Porter Road in Lake Wales, was transported to Bartow Regional Medical Center but succumbed to his injuries and was declared deceased. He was a student at Winter Haven High School. He was wearing a seatbelt.

The Chevy 3500 flatbed truck had four occupants:

Driver – 35 yr old Liopoldo Cisneros of Avon Park – transported to Bartow Regional Medical Center, non-life-threatening injuries;

Front Seat Passenger – 30 yr old Vincente Vasquez of Avon Park – transported to Bartow Regional Medical Center, non-life-threatening injuries;

Back Seat Passenger – 33 yr old Alfredo Cisneros of Avon Park – transported to Bartow Regional Medical Center, non-life-threatening injuries;

Back Seat Passenger – 29 yr old Gilberto Avilla of Avon Park – transported to Lakeland Regional Health Medical Center, non-life-threatening injuries;

The investigation is ongoing. Neither excessive speed nor impairment on the part of the surviving driver appear to be a factor. At the time of the crash it was dark with some fog present.

Bartow Rotary Clubs present Yellow Jacket Stomp Adventure Run V

The 5th annual Yellow Jacket Stomp Adventure Run will be held in Mosaic and Mary Holland Parks at 10:00 a.m. on Saturday, February 4, 2017. 
The course is run over a combination of grass, paved streets and walking paths.  Various obstacles will be arranged to challenge the runners on their way around the course.

The registration, start, finish and awards ceremony will be held in Mosaic Park at 2250 South Floral Avenue in Bartow, one block north of the Bartow Civic Center. 

The fees are $20 for runners ages 16 and up/ and $15 for runners ages 15 and under.

BARTOW, Fla. (December 28, 2016) – Polk County Fire Rescue crews worked several significant structure fires this holiday season that led to death of several animals and destruction of three homes.
 
On Christmas day, two dogs were killed in a mobile home fire in North Lakeland. Around 5:30 p.m. Polk County Fire Rescue crews arrived at 1721 Quail Trail where smoke billowed from both ends of a mobile home. Crews entered it to see if anyone was trapped inside. But what they found were two unresponsive dogs located under a bed. Attempts to revive the dogs were unsuccessful.
 
The Red Cross was contacted to assist the person who lived at the residence with temporary housing and food. The person who lived at the home told crews there was faulty wiring in the home, but the cause of a fire has been ruled undetermined by Polk County Fire Rescue.
 
The following day, a home in Indian Lake Estates was destroyed by fire. Firefighters arrived about 6:45 a.m. at 2600 Park Ave in Lake Wales to find heavy smoke and some flames coming from a single story home.
 
While crews were assembling, bystanders told them an elderly man lives in the home. But upon searching the home, firefighter crews found no one was inside. At that point, the fire started to vent through the roof. Crews exited the home and attacked the home from the outside. No one was injured.
 
Tuesday, one person was injured and two dogs died due to a structure fire. Just after 1 p.m. firefighter crews were dispatched to 521 Woodland Ave. in Lakeland. Crews were able to quickly stop the fire from spreading to a nearby trailer park. One of the homes nearby received some damage from the fire.
 
Four adult males and one adult female, who lived at the home, were displaced and the Red Cross was called to assist with food, clothing and shelter.
 
It is thought the fire started in the kitchen area, though the exact cause is not clear. Polk County Fire Rescue turned over the investigation to the State Fire Marshal’s Office.

Bartow, Florida – According to court reports Malquain Eason who had been sought for attempted murder was arrested yesterday. He had a first appearance today. Details have not been released at the time of this article if he has been issued bond. He has another hearing scheduled for January 31, 2017.

On 12/21/16 at approximately 5:00PM, officers with the Bartow Police Department responded to the area of 1050 S Golfview Ave. in reference to a fight and shots being fired. While on scene, it was learned that a victim had been transported to the Bartow Regional Medical Center, for two apparent gunshot wounds. The investigation revealed that the suspect and victim were engaged in an argument which turned violent. The suspect then pulled a semi-automatic handgun and fired several shots at the victim. After the shooting the suspect fled the area. The victim, 19 year old Herbie Dungy of Bartow, was treated for injuries related to the shooting and released from LRMC.

Bartow, Florida – – Dangerous levels of carbon monoxide from a mechanical failure in an HVAC/exhaust system at LeadFeather Guns & Archery led to the fainting of two people Monday, December 26, 2016.

Polk County Fire Rescue crews were dispatched about 3:30 p.m. Monday to 410 Overlook Drive, Winter Haven, to assist a patient who had fainted. The address is an indoor shooting range. Initially the patient refused treatment/transport, but before crews left the scene, another patient fainted. Crews then used a carbon monoxide monitor and found dangerous levels in the building.

Polk Fire’s special operations unit was called while the building was evacuated. The building was then ventilated until levels were deemed safe.

Later it was determined a mechanical failure in the HVAC/exhaust system that services the indoor gun range caused the carbon monoxide buildup. Due to the increased shooting activity on Monday, this allowed the concentration of gunfire smoke and carbon monoxide to quickly build up in the range area. While a number of people were affected by the increase of carbon monoxide, three were treated at the local hospital.

The gun range portion of the facility is expected to reopen as soon as repairs to the HVAC/exhaust are completed. The range’s management is concerned about this incident and is working with Polk Fire inspectors and Polk County building officials to determine a solution that would ensure such episodes do not occur in the future.

Polk Fire’s fire prevention inspectors were on scene on Tuesday to survey the situation.

Judy Underwood, 70, of Florida, formerly of Danville, passed away December 21, 2016 in Florida. She is survived by her loving husband of 55 years, Donald Underwood. Judy was born on April 9, 1946 in Danville, Kentucky to Laura Reece Campbell Collins and the late James Campbell.
 
Judy worked for the State Hospital on Burgin Road for many years and retired as a ward clerk for Eastern State Hospital in Lexington. Judy loved to fish,cook for her family, Dale Earnhart, Jr. and being a member of her neighborhood patrol for SunRay subdivision in Frostproof, Florida. Most of all, she loved her family dearly, raising four children and two grandchildren.
 
Also surviving are four children: Janie Robbins (Johnny) of Danville, Donna Prewitt of Danville, James Clay “Sonny” Underwood (Tracey) of Emmaus, Pennsylvania, and Robert Underwood of Frostproof, Florida; two grandchildren: the Jennifer LeeAnn Robbins and Dathan Nathaniel Prewitt, both of Danville; and a special sister-cousin, Jo Ellen Brown of Junction CIty.
 
Along with her father, she is preceded in death by two grandchildren, Aleshia Jane Robbins and Michael Lee Robbins and two sisters: Sharon Smith and Minnie Ann Campbell.
 
Funeral services will be 11AM Wednesday, December 28, 2016 at Preston Pruitt Spurlin Funeral Home with Bro. Odis Clark officiating. Burial will be in Junction City Cemetery. Visitation will be Tuesday, December 27, 2016 from 5-8PM.
 
Pallbearers are Randy Noel,Brian Campbell, Kyle Curtis, Nate Smith, Terry Honaker, Chris Underwood, and Jared Reinsmith.

Employee of the Year is Annette Baldree, RN, Infection Control.

“Annette has achieved respect from all levels of staff at LWMC, including physicians,” her supervisor said. “Since the beginning, Annette has ‘taken the bull by the horns.’ Staff comes to her anytime there are questions concerning various disease processes discovered in our patient populations to provide information on the delivery of care for preventing the spread of infections. She has established strong community relationships, particularly with the local department of health epidemiologists. She is a stellar performer.”

Lakeland, Florida -LAKELAND, FL (December 28, 2016) – The Lakeland Police Department responded to a report of domestic violence at the Westlake Apartments, 501 Hartsell Avenue, at approximately 11:00 p.m. on Tuesday, December 27, 2016. The victim advised officers that her boyfriend, Nelson Santiago, age 44, punched her in the face during an argument and then fled the apartment. Witnesses at the location reported seeing Santiago holding a handgun as he fled.

LPD officers, with the assistance of the Polk County Sheriff’s Office helicopter, searched the area for approximately one hour but were unable to locate Santiago and cleared the scene. Approximately 30 minutes later, Lakeland Police Department received a call that Santiago had returned to the apartment. First responding officers observed Santiago in the apartment and set up a perimeter, considering him armed and dangerous.

While on scene at the standoff, officers learned that Santiago had pointed a firearm at another Westlake Apartments resident at approximately 8:45 p.m. on December 27th. According to the victim, Santiago was brandishing a long sword and a handgun, which he pointed at her, as she was driving into the complex. The victim positively identified Santiago out of a photo pack and detectives were able to secure a warrant for Aggravated Assault with a Deadly Weapon.

After several hours of unsuccessful attempts to convince Santiago to surrender peacefully, the SWAT team and agents with the Bureau of Fire and Arson Investigations responded to the scene. After no verbal response from Santiago, officers deployed chemical agents into the apartment. Within minutes Santiago attempted to flee out the front door of the apartment and was apprehended by Police K9 Valor. Santiago was transported to Lakeland Regional Health and treated for injuries sustained during the K9 apprehension. When cleared, he will be transported to Polk County Jail and booked on felony charges of Domestic Violence and Aggravated Assault.
