Lake Wales, FL – Jeff Bassett, Maintenance Engineer in Plant Operations, was named October Employee of the month at Lake Wales Medical Center. Jeff has worked for the hospital for the past three years.

 

“Jeff is committed to his job; he makes sure that the job is done safely and correctly,” a co-worker wrote in nominating him. “He takes the extra step to ensure that a problem won’t happen again. He is always willing to help his co-workers or visitors when he sees that they need help. The hospital will hold a free information sessions at Lake Wales Care Center’s Community Room. 140 E. Park Ave., on Tuesday, Nov. 29 at 6 p.m. Certified Assisters will be on-site to share information, answer questions, and to schedule free appointments for application assistance. Refreshments will be served.

 

Open Enrollment opened Nov. 1 and continues through Jan. 31. This marks the federal government’s fourth annual campaign to extend healthcare coverage to more Americans, specifically to those who otherwise wouldn’t have access to services or who would face a financial crisis due to medical bills.

 

“We have helped hundreds of individuals enroll on the Marketplace since it first launched four years ago,” said Rebecca Brewer, CEO of Lake Wales Medical Center. “It’s very gratifying to help individuals enroll knowing they will be able to access healthcare when they need it. It’s a relief for so many people.”

 

Lake Wales Medical Center also is offering free one-on-one assistance throughout the Open Enrollment period at the hospital. Certified assisters will help with plan comparisons, determining any tax credits or discounts, and filling-out applications.

Ingredients

1 (10.75 ounce) can condensed cream of mushroom soup
1 cup mayonnaise
1 egg, beaten
1/4 cup finely chopped onion
3 (10 ounce) packages frozen chopped broccoli
8 ounces shredded sharp Cheddar cheese
salt to taste
ground black pepper to taste
2 pinches paprika
Add all ingredients to list

  1. Preheat oven to 350 degrees F (175 degrees C). Butter a 9×13 inch baking dish.
  2. In a medium bowl, whisk together condensed cream of mushroom soup, mayonnaise, egg and onion.
  3. Place frozen broccoli into a very large mixing bowl. (I like to use my large stainless steel bowl to mix this recipe thoroughly.) Break up the frozen broccoli. Using a rubber spatula, scrape soup-mayonnaise mixture on top of broccoli, and mix well. Sprinkle on cheese and mix well. Spread mixture into prepared baking dish, and smooth top of casserole. Season to taste with salt, pepper and paprika.
  4. Bake for 45 minutes to 1 hour in the preheated oven.

Frostproof Man’s Death While An Inmate In South County Jail Being Investigated

Frostproof, Florida – Polk County Sheriff’s Office detectives are investigating the death of a Polk County Jail inmate who was found unresponsive in an isolation cell at approximately 12:40 am, Saturday, November 19, 2016.  

23-year-old Brandon Lee Levan of Frostproof was jailed on charges of Simple Assault/Threat to do Violence, Unarmed Burglary of Occupied Dwelling, and Impersonating a Law Enforcement Officer.  He was arrested in September 2016.  At the time of his arrest, Levan was baker acted and was subsequently housed by himself in SMED Unit under a suicide watch.   

While conducting routine rounds, detention deputies observed Levan lying, face up, in his cell and appeared to be sleeping.  On a second round, within 12 minutes of the previous check, a detention deputy attempted to verbally wake Levan but was unsuccessful.  Detention deputies then entered the cell – found Leven physically unresponsive and CPR was immediately started.  An AED was utilized and a shock was not advised. Detention deputies continued CPR until EMS arrived on scene and took over. Polk County Emergency Services arrived and continued to render aid.  Levan was pronounced deceased by EMS at approximately 1:19 am.   

There was no apparent trauma.  An autopsy will be conducted to determine the cause and manner of death.  

Levan had Exostosis, which is an abnormal formation of a bony growth on existing bones.  Levan’s medication was administered via injection by jail medical personnel.  Levan was baker acted in 2013, 2015, and again at the time of his arrest in September 2016.  During the screening process as he was being booked into the jail in September, Levan admitted to regular methamphetamine use, K-2 use, and alcohol abuse.     

Levan’s criminal arrest history includes charges of DWLSR in February 2015 – citation given; Possession of Xanax, and Possession of Drug Paraphernalia in April 2016- booked into jail; and as previously mentioned, he was arrested for Simple Assault/Threat to do Violence, Unarmed Burglary of Occupied Dwelling, and Impersonating a Law Enforcement Officer in September 2016.  

This is an ongoing death investigation. It is standard PCSO policy for four separate mandated, independent investigations to be conducted when an inmate dies. The PCSO Administrative Investigations Section is conducting an internal review. The PCSO Bureau of Criminal Investigations is conducting the death investigation. The Polk County Medical Examiner will conduct the autopsy to determine the manner and cause of death. The State Attorney of the 10th Judicial Circuit will review the administrative and criminal investigation, as well as the autopsy information, and conduct their own separate investigation.

Polk County Deputies Investigating Possible Stabbing Homicide

Lakeland, Florida – Polk County Sheriff’s 0ffice Homicide detectives are conducting a suspicious death investigation at 12505 Country Place Road, in Lakeland.

A call was received at approximately 10:53 pm, on Friday, November 18, 2016, reporting a possible stabbing at the aforementioned address.

Upon their arrival, deputies located 32-year-old Gabriel Gamino, Country Place Road, Lakeland, with an obvious injury to his face.  Gamino was transported to Lakeland Regional Health where he later died at 11:50 pm.

Through the investigation, detectives learned Gamino had been in an altercation with an unknown male in Gamino’s front yard.  The unknown male struck Gamino then fled the area.  Gamino went to a neighbor’s house for assistance where he collapsed.

Polk State Names Andy Oguntola Director of JD Alexander Center In Lake Wales

Lake Wales, Florida – Polk State College has named Andy Oguntola as director of the Polk State JD Alexander Center in Lake Wales.

A Lake Wales resident, Oguntola comes to Polk State from Valencia College, where he served as director of admissions and registration and as adjunct professor.
Oguntola earned a bachelor’s degree and a Master of Business Administration from Webber International University. He is working toward a doctorate in education from Nova Southeastern University.
“Living in Lake Wales for the last 15 years, I’ve seen the ability of Polk State to grow,” says Oguntola.
“I’ve seen what the Polk State JD Alexander Center does for the city of Lake Wales, and I want the Center to not only be in the city but also to become of the city.
“I believe I can bring my skills and talents to be strategic and create successful relationships for students at Polk State.”
In his new position, Oguntola plans to focus on promoting higher education in general and Polk State in particular.
“One of my biggest goals is to make sure that every student in Lake Wales has a chance to go to college,” he said.
“That’s my main focus — to make sure we get in the community and show these kids that Polk State is right in their backyard.”
Achieving that goal, said Oguntola, will require a team effort.
“I call it a team, but we’re really a family. I want to inspire this group of individuals and these leaders to push ourselves to the maximum potential and to grow this campus to its fullest potential.”
As a Lake Wales resident who is active in his community, Oguntola looks forward to partnering with local leaders.
“I have strategic relationships in the city and I know a lot of individuals and believe we can sit down and have conversations and really try to figure out ways that we can work together to create successful opportunities for students.
“I also want to focus on parents because as they learn what Polk State offers, they will encourage their kids to go to college and some of those adult learners will also come to college. We’ll try to bring both students and parents to college.”
Local leaders in Lake Wales also look forward to working with Oguntola.
“We are pleased to hear that Mr. Oguntola was selected as the new director for the Polk State JD Alexander Center,” said Jesse Jackson, superintendent of the Lake Wales Charter Schools.
“Since opening its doors, the Polk State JD Alexander Center has become an important higher education resource for the Lake Wales community. Winter Haven Police Investigating A Possible Homicide After Car Rolls Into Apartment

Winter Haven, Florida – On 11-18-16 at 1:28 a.m. a 9-1-1 call was recevied initially as a vehicle crash into an apartment located at 645 Ave I Northwest in Winter Haven.

The caller stated that a vehicle rolled into the back of her apartment from an adjacent parking lot with what appeared to be a man who was passed out. When officers arrived, 39 year-old Jimmie Lee Rogers, Jr was found unresponsive with what appeared to be a gunshot wound.

EMS responded and transported Rogers to Winter Haven Hospital where he was declared deceased. At this time, investigators are working this scene as a homicide.

The victim’s family has been notified and responded to the hospital.

Polk Sheriff’s Department Searching For Escaped Trustee

UPDATE – Polk County Sheriff’s Office deputies located and arrested escaped trusty inmate 45-year-old Joseph Flanagin.

Flanagin was located in the Providence and Kathleen Road area of north Lakeland.  He is custody.  He faces charges of Escape and Grand Theft Auto.

Polk County Sheriff’s Office deputies are conducting a search for inmate trusty, 45-year-old Joseph Flanagin of Lakeland. Flanagin was last seen by Polk County BoCC employee at 3:30 pm at the county’s fleet maintenance building located at 2490 Bob Phillips Road, in Bartow.  Flanagin took a white Ford pick-up truck but the truck has since been recovered.
Flanagin was arrested on September 22, 2016 charged with VOP Petit Theft 3rd Conviction, and was sentenced to 300 days in the county jail – he had already served 66 trusty days.

Flanagain is described as being a white male, six feet tall and weighing approximately 180-200 pounds.  He has brown hair color with a shaved head and hazel eyes.  Flanagin is wanted for vehicle theft, and escape.

This is an active an ongoing investigation, when there is additional information to release it will be forwarded via media alert.
